In-hospital safe glycaemic control
In-hospital hyperglycaemia has been found to be an important marker of poor clinical outcome and mortality among diabetic patients. The in-hospital care application domain of the REACTION platform features a range of services aiming at safe glycaemic control of patients with diabetes using an individual target level, depending on the history and actual state of the patient.
In the general ward, a REACTION application monitors a range of parameters including glucose level, nutritional intake as well as measures of administered drugs (e.g. oral anti-diabetic drugs or insulin) among other parameters (e.g. insulin sensitivity). The data is contextualised in the Data Management component and mathematical algorithms will later be used to calculate the required insulin doses. Results is delivered to clinicians for validation and verification. Their on-line appraisal can be fed back to the physicians and nurses at the point of care in the patients ward. For the implementation of the new REACTION application, interfaces to hospital systems will be developed and hospital protocols across wards for administration and glucose management will be required.
